When someone you love begins to need more care, knowing where to start can feel overwhelming. I founded SW Life Transitions & SW Transition Solutions because I saw too many families struggling to navigate the complicated search for assisted living and memory care while trying to care for the people they love.

As a Certified Senior Transition Specialist, former nurse, and licensed real estate professional, I serve as your advocate throughout the process. I'll help assess your loved one's needs, compare communities, research quality and compliance records, and connect you with trusted resources, all at no cost to your family. You remain in control of every decision while I handle the research and planning.

My mission is simple: give families more time with Mom, Dad, or their loved one while providing guidance rooted in empathy, transparency, and trust.
